2024/2025 Registration Form
Please submit this registration form along with mailing a $20 check (non-refundable) made payable to Milford Dance Center. You may also Venmo (milforddancecenter) or PayPal (milforddancecenter@yahoo.com) your fee.
Make sure to fill in all contact information.
Registration fees are per child, and for fall registration only.
No registration fee required for summer classes
*A confirmation email will be sent after we receive your child's registration. If you do not receive an email within
